What happens after you submit
Submit the referral
Complete the online form with coordinator, consumer, and point-of-contact information. Attach the IPP, facesheet, or CDER if you have them ready.
We confirm receipt
You receive an emailed confirmation with a reference number immediately, and our intake team reviews the referral the same business day.
Intake and matching
We contact the family, confirm authorized hours and language needs, and match a trained caregiver to the household.
Care begins
Shifts start on the authorized schedule with real-time clock-in, task logging, and guardian sign-off after every visit.
